The CDS obtains appropriate clinical documentation through collaborative communications from having developed positive working relationships with physicians, nursing staff, other patient care providers, and the Health Information Management Department Coders. The CDS is also responsible for providing informal and formal education to all members of the care team regarding clinical documentation.     Clinical Documentation Specialist Education and Experience Requirements:  - Must have a Bachelor’s degree in Nursing from an accredited academic body. - Must be actively licensed as a Registered Nurse within the state of Illinois. - Must have a minimum of five years' progressive work experience working as a Registered Nurse. - Must have basic computer skills in word processing and spreadsheet utilization. - Must have excellent interpersonal skills necessary to develop and foster effective physician communication. - Must have the analytic skills necessary to clinically assess medical records. - Must have excellent prioritization and organizational skills. - Holding the CCS coding credential is preferred. - Holding a Master’s degree in Nursing or another healthcare-related field is preferred. - Experience working in Case Management, Med/Surg, or Critical Care Nursing preferred.

Bring Your Passion to the Forefront:  Practice Support Coach Work location will be at Ingalls office in Harvey.   As an integral member of the Center for Transformative Care, the Practice Support Coach will work with primary care practices to improve performance in quality of care. Using population registries and related data, Practice Support Coaches work with their assigned practice teams to ensure that patients receive the condition-specific services they need. For example, Practice Support Coaches will: coordinate proactive patient outreach to schedule follow up appointments, screen/monitor patients using questionnaires regarding mental health or other conditions, coach patients as they prepare for cancer screening procedures, and provide reminders about their pre-visit lab tests.    Practice Support Coaches are also expected to monitor and extract reports from patient registries and share population level data and outcome measures on a regular basis. S/he will support primary care physicians and practices in managing their panel of patients using a registry population management informatics tool. By gathering and organizing patient data, the Population Health Coordinator works to identify patients’ unmet needs, engage patients in their own care, gather summary information for treatment interventions, and enhance ongoing communication between the patient and her/his care team.     Practice Support Coach Qualifications  - Medical Assistant (MA) or equivalent experience - Minimum of 3 years’ experience in in a health care setting. This director will be responsible for the accuracy and timely reporting of UCMs financial statements including the day-to-day control of all accounting and financial reporting operations and financial records.
