Join a world-class academic healthcare system, UChicago Medicine, as a CCHA Nurse Navigator in the Pediatric Department.
Under the direction of a faculty sponsor and in collaboration with Chicagoland Children’s Health Alliance (CCHA) Leadership, coordinates the care of patients through the disease process across the CCHA network. Acts as a clinical liaison for patients, across medical services, clinical departments, and facilities.
Essential Job Functions:
Serves as a clinical partner to facilitate a seamless patient experience across the CCHA network.
Coordinates appointment with program providers (see “Specific Departmental Requirements” below).
Oversees the scheduling of new patient’s initial appointment with the appropriate provider across CCHA
Develops and/or coordinates development of system education materials, clinical pathways, and quality studies.
Escalates or directs barriers to access for patients across the CCHA Network
Coordinate with care teams to ensure patient appointments are within 7 days.
Serves as call center contact and escalation point for intake coordinators
Coordinates appointments and facilitates follow testing
Performs other duties assigned.
Required Qualifications:
Minimum of two years in care coordination, nurse navigation, or case management role.
Preferred Qualifications:
Pediatric GI experience preferred.

Compensation & Benefits Overview
UChicago Medicine is committed to transparency in compensation and benefits. The pay range provided reflects the anticipated wage or salary reasonably expected to be offered for the position.
The pay range is based on a full-time equivalent (1.0 FTE) and is reflective of current market data, reviewed on an annual basis. Compensation offered at the time of hire will vary based on candidate qualifications and experience and organizational considerations, such as internal equity. Pay ranges for employees subject to Collective Bargaining Agreements are negotiated by the medical center and their respective union.
