Job Description

Be a part of a world-class academic healthcare system, UChicago Medicine Ingalls Memorial Hospital as an Department of Human Services (DHS)/ Fitness Restoration Program Coordinator in theBehavioral Health department. This position is a 100% onsite opportunity. You will need to be based in the greater Chicagoland area.

 

The DHS Fitness Restoration Program Coordinator provides general oversight of the specialty Fitness Restoration Program in collaboration with the Nursing Patient Care Manager. The Fitness Restoration Coordinator ensures compliance with quality and regulatory standards, communication with the Department of Human Services (DHS) and management of the legal processes.

 

Essential Job Functions

  • Collaborates with Behavioral Health Leadership to ensure all regulatory compliance, clinical competency and educational requirements are met
  • Creates a learning environment that encourages staff to maximize level of professional practice
  • Designs and continually evaluates core competencies int he implementation of a Therapeutic community model of care
  • Regularly meets with the patients community  leaders to promote a  therapeutic, healing and empowering culture
  • Acts as the primary facilitator for daily community meetings
  • Ensures the behavioral incentive program for DHS patients is functional and consistently maintained
  • Ensures patients have a minimum of one hour time outdoors (weather Permitting)
  • Observes Clinical Therapist, RN an BHT lead groups, and provides feedback and education to build skills as appropriate
  • Audits EMR for quality, compliance with group programming and completeness for clinical documentation
  • Designs and implements a professional development program for core DHS staff
  • Promotes a culture of Employee Engagement
  • Facilitate appropriate admissions tot he program with the goal of stabilizing the unit census
  • Coordination with the accepting attending Psychiatrist and nursing staff regarding the needs of the patient
  • Communication with the referring county authorities to plan transportation to DHS program, which includes establishing the transport date and accurately composing the transport authorization letters
  • Review referral packets from DHS for individuals determined to be Unfit to Stand Trial (UST)
  • Participate in the weekly meetings with DHS leadership

Required Qualifications

  • Master's Degree in Clinical Behavioral Health 
  • Minimum of 3 years' experience in a Behavioral Health Care environment, preferred inpatient BH, Forensic Clinical experience
  • Licensed Clinical Social Worker (LCSW)
  • Current BLS with American Heart Association 
  • Current CPI

Preferred Qualifications

  • Licensed Clinical Professional Counselor (LCPC)

Compensation & Benefits Overview

 

UChicago Medicine is committed to transparency in compensation and benefits.  The pay range provided reflects the anticipated wage or salary reasonably expected to be offered for the position.

 

The pay range is based on a full-time equivalent (1.0 FTE) and is reflective of current market data, reviewed on an annual basis. Compensation offered at the time of hire will vary based on candidate qualifications and experience and organizational considerations, such as internal equity. Pay ranges for employees subject to Collective Bargaining Agreements are negotiated by the medical center and their respective union.
