Join a world-class academic healthcare system, UChicago Medicine Crown Point, located in Crown Point, Indiana as a Breast Imaging Nurse Navigator (RN). UChicago Medicine is an integrated academic and community health system with multiple primary medical facilities within the surrounding suburbs of Chicago. UChicago Medicine, Crown Point is a two-story, 130,000-square-foot multispecialty care center and micro-hospital in Northwest Indiana, the academic health system's largest off-site facility and its first freestanding building in Indiana.
The Breast Imaging Nurse Navigator (RN) provides individualized assistance to patients, families, and caregivers to help overcome health care system constraints and facilitate timely access to quality medical and psychosocial care from screening and pre-diagnosis of cancer through placement into appropriate treatment clinics. The Breast Imaging Nurse Navigator works closely with Physicians and staff in all specialty areas to coordinate the patients plan of care and communicates with all members of the team on behalf of the patient.
Essential Job Functions:
Coordinate appointments for consults with support services within established service standards, including socialworkers, dieticians, genetic counselors, therapists, and others when needed.
Collaborate with DI Operations, access and other teams to obtain medical records, imaging/pathology, and insurance information to expedite initial consultation.
Collaborates with clinic staff, research nurses, and physicians to ensure appropriate Breast Imaging for potential and eligible clinical trial patients.
Apply knowledge of payer requirements and insurance processes and their impact on referrals for treatment and patient care decisions to set up appropriate treatment, as requested or needed.
Responds to voicemail and myChart messages within one business day and answers phone calls in a timely manner.
Other duties as assigned, including other leadership roles as required by the center.
Required Qualifications:
National certification in Oncology and specialty practice area (e.g., CBCN, OCN, ONN-CG) preferred.

Compensation & Benefits Overview
UChicago Medicine is committed to transparency in compensation and benefits. The pay range provided reflects the anticipated wage or salary reasonably expected to be offered for the position.
The pay range is based on a full-time equivalent (1.0 FTE) and is reflective of current market data, reviewed on an annual basis. Compensation offered at the time of hire will vary based on candidate qualifications and experience and organizational considerations, such as internal equity. Pay ranges for employees subject to Collective Bargaining Agreements are negotiated by the medical center and their respective union.
