Job Description

Be a part of a world-class academic healthcare system at UChicago Medicine as a Nurse Navigator with our Heart & Vascular Team in Hyde Park. In this role you will partner with the rest of the clinical team to provide patients the highest quality of care. 

 

Under the direction of the Advanced heart transplant providers and in collaboration with the program support team, coordinates the care of patients through their post heart transplant care. Acts as a clinical liaison for patients going through this process in the clinic and in the in-basket messages.  Assesses clinical, emotional, spiritual, psychosocial, financial, and other patient needs. Evaluates and manages patient symptoms post heart transplant.

Essential Job Functions:

  • Provides care coordination & patient education to patients in heart and vascular service line. 
  • Facilitates resolution to patient care gaps and monitors follow up/completion by both proactively reaching out to patients and addressing direct phone calls and messages from patients.
  • Acts as liaison for patients, families and their multidisciplinary clinical team. 
  • Educates/reinforces education with patients and families on disease, treatments, side effects, adverse reactions, and reportable signs/symptoms based on multidisciplinary care plan. 
  • Develops and/or coordinates development of patient education materials.
  •  Serves as primary point of contact for patients discharged from the hospital to address questions and patient care needs to prevent readmission or an emergency room visit.
  • Attends clinic with providers, may include scheduling procedures, testing, and educational information for a successful procedure.
  • Attends transplant meetings and advanced heart failure meetings.
  • Works in collaboration with other nurses in the advanced heart failure /trp program.
  • Performs other duties assigned.

 

Required Qualifications

  • Registered Nurse with a current Illinois license. 
  • Minimum of two years clinical nursing experience with demonstrated clinical competence in patient care, teaching and management of patients in cardiology. 
  • Must have general computer skills and be adaptable to changing working situations and work assignments. 
  • Excellent interpersonal skills required, with specific ability to communicate effectively with patients, family members, and physicians. 

 

Preferred Qualifications

  • 2 years of Cardiac  experience
  • Experience cardiac procedures and heart transplant preferred

Compensation & Benefits Overview

 

UChicago Medicine is committed to transparency in compensation and benefits.  The pay range provided reflects the anticipated wage or salary reasonably expected to be offered for the position.

 

The pay range is based on a full-time equivalent (1.0 FTE) and is reflective of current market data, reviewed on an annual basis. Compensation offered at the time of hire will vary based on candidate qualifications and experience and organizational considerations, such as internal equity. Pay ranges for employees subject to Collective Bargaining Agreements are negotiated by the medical center and their respective union.
