Job Description

Join UChicago Medicine, as an Advanced Practice Provider within the Lung Transplant department. At the core of our lung transplant program is a team of internationally known thoracic surgeons who are leaders in the field and are often among the first to perform groundbreaking procedures.

As an Advanced Practice Provider, you will be responsible for comprehensive direct patient care management and OR surgical assist. In this role, the APP will manage a caseload of patients and exercise autonomous decision making in the assessment, diagnosis and initiation of delegated medical treatments. The scope of practice includes operating room support, consultation, patient management, discharge planning, staff education, consultation and research. The APP will utilize advance practice skills obtained through the completion of a formal provider program. These skills include advanced levels of assessment, planning, intervention, and evaluation of care. The APP is also responsible for planning, organizing, and directing patient care efforts in collaboration with medical and surgical physicians and other team members.

 

Essential Job Functions

 

Advanced Practice Provider – Lung Transplant

The Advanced Practice Provider (APP) delivers comprehensive care to lung transplant patients across the continuum, including management in the intensive care unit, the general inpatient floor, and during readmissions occurring immediately post-transplant or years later.

Key Responsibilities:

  • Perform systematic patient assessments, integrating history, diagnostic testing, physical examination, and psychosocial evaluation.
  • Differentiate normal and abnormal findings, accurately interpret diagnostic studies, and develop problem lists with associated differential diagnoses.
  • Formulate, implement, and evaluate individualized care plans in collaboration with patients, families, physicians, nursing, and the multidisciplinary team.
  • Perform routine diagnostic and therapeutic procedures and manage pharmacologic therapies in accordance with established protocols and standardized procedures.
  • Coordinate care transitions including discharge planning, referrals, follow-up, and ongoing health maintenance.
  • Present and document clinical assessments, impressions, and plans accurately and in a timely manner.
  • Provide patient- and family-centered care that addresses the physical, psychosocial, and developmental needs of adolescent, adult, and geriatric populations.
  • Participate in research, quality improvement, and evidence-based practice initiatives related to lung transplantation.

Nurse Practitioner Education and Experience Requirements:

  • Masters of Science in Nursing
  • Current and/or eligibility for State of Illinois RN and APN licensure/registration
  • Must be certified as a Nurse Practitioner by the ANCC or AANP
  • Must have or be able to qualify for Illinois and DEA prescribing numbers
  • At least 1 year of inpatient nursing experience preferred but not required
  • Critical Care experience preferred

Physician Assistant (PA) Required Qualifications

  • Completion of an accredited (Physician Assistant) educational program
  • Certification by the National Commission on the Certification of Physician Assistants (NCCPA)
  • Current and/or eligibility for State of Illinois licensure/registration
  • Certification in ACLS
  • Critical Care experience preferred

Compensation & Benefits Overview

 

UChicago Medicine is committed to transparency in compensation and benefits.  The pay range provided reflects the anticipated wage or salary reasonably expected to be offered for the position.

 

The pay range is based on a full-time equivalent (1.0 FTE) and is reflective of current market data, reviewed on an annual basis. Compensation offered at the time of hire will vary based on candidate qualifications and experience and organizational considerations, such as internal equity. Pay ranges for employees subject to Collective Bargaining Agreements are negotiated by the medical center and their respective union.
