Job Description

Be a part of a world-class academic healthcare system, UChicago Medicine, as an Staff Pharmacist in the Pharmacy Department.

 

Responsible for the provision of clinical and distributive pharmacy services for patients receiving treatment within his/her assigned area.  Pharmacists are aligned with a medical service or group of patients based on a geographical area within the University of Chicago hospitals.  To provide active participation in related education and research activities.

 

Essential Job Functions

  • Ensure the accuracy and appropriateness of all medication orders by evaluating drug selection, dose, frequency, safety, dosage form, allergies, drug interactions, and disease state interactions prior to dispensing of medications.
  • Ensure the accuracy and appropriateness of all medication orders prior to dispensing by evaluating patient information including (but not limited to) age, weight, relevant medical characteristics, and laboratory results. 
  • Demonstrate the knowledge and skill necessary to provide care appropriate to the age of assigned patients.
  • Demonstrate knowledge of the principles of growth and development over the patient’s life span and assess data reflective of the patient’s status.
  • Interpret the appropriate information needed to identify each patient’s requirements relative to his or her age-specific needs and provide the care needed.
  • Supervises pharmacy technicians working under their direction to provide the following and assure the completion of:
    • Preparation of first and missing doses
    • Triaging the order of the work flow
    • Order Entry
    • Delivery of STAT medications as necessary
    • Short-list preparation and stock replacement
    • IV Therapy coordination
    • Replacement of CPR car as needed to nursing unit
    • Nursing unit inspections, checking for expired medications, refrigerator checks, hood cleaning, and other responsibilities as defined.
    • Replacement of PRN’s and floor stock
    • Cassette delivery
    • Maintaining the general cleanliness of the area
    • Other activities related to medication dispensing
  • Identify service requirements by interaction and consultation with patients, physicians, nurses, pharmacists, and technicians
  • Ensure proper drug utilization by monitoring compliance with drug use criteria, clinical pathways, and IV to PO conversion. 
  • Contribute to ongoing drug utilization review activities and other quality assurance reporting programs by active support and participation
  • Participate in the department’s chemotherapy quality assurance program which includes, but is not limited to, preparation of chemotherapeutic medications, monitoring of patients receiving chemotherapy, ensuring that all chemotherapy regimens are appropriate for individual patients based on disease, organ function, height and weight, body surface area calculations, age, etc.
  • Exercises professional judgment in evaluating the safety and efficacy of all medication orders written for this patient population.
  • Is the primary resource person for drug information for this patient population and is actively involved in educating patients regarding their drug therapy.
  • Ensure proper dosing of medications based on Pharmacokinetic principles and serum drug level monitoring.
  • Participation in the hospitals Pharmacokinetic dosing service
  • Document clinical interventions which prevent or eliminate drug-related problems (i.e. Adverse drug reactions, drug interactions, etc.) or promote cost-effective drug therapy
  • Provide consultation for assigned patient populations and as requested by the medical staff.  Educate patients on safe and effective use of medications, possible side effects of their medications, and the management of these side effects
  • Clarify and solve problems concerning medication orders with prescribing physicians, suggest alternative medication therapies to physicians when appropriate, serve as a source of information for Investigational drug information for health professionals, identify and report adverse drug reactions
  • Communicate relevant finding or recommendations to other health care professionals by direct verbal communication or by providing written documentation in the patient’s medical record when requested by the physician
  • Maintain and enhance up-to-date professional knowledge of drug and drug therapy by attending educational meetings and reviewing relevant professional journals and suggested readings
  • Assist in the orientation and training of new personnel
  • Participate in ambulatory pharmacy dispensing activities as required.  Perform dispensing, counseling, and clinical services in the outpatient setting as their practice site
  • Performs other duties as requested by senior management

Required Qualifications

  • Graduate of an ACPE accredited School of Pharmacy
  • Registered pharmacists in the state of Illinois.
  • Exceptional aptitude or knowledge may be considered in lieu of experience.
  • Knowledge of contemporary hospital/clinical practice and service.
  • Strong written and verbal communication skills.
  • Ability to interact constructively with supervisors, peers, support personnel, and students.
  • Knowledge and understanding of general hospital operations.
  • Ability to integrate clinical services, distributive services, education, and research activities

Preferred Qualifications

  • Doctor of Pharmacy or another advanced degree
  • Completion of an ASHP accredited residency 
  • Two years hospital pharmacy experience in an organized health care setting

In this role, we anticipate that you will generally work approximately under 40 hours per week.  We anticipate that you will generally be scheduled to work the day shift, 7am-8pm- rotating and will not be scheduled for on-call shifts. We anticipate you will be scheduled to work 100% at the Calumet City campus. The days of the week that you are scheduled will be Monday-Friday, working every other weekend.

UChicago Medicine is committed to transparency in compensation and benefits.  The pay range provided reflects the anticipated wage or salary reasonably expected to be offered for the position.

 

The pay range is based on a full-time equivalent (1.0 FTE) and is reflective of current market data, reviewed on an annual basis. Compensation offered at the time of hire will vary based on candidate qualifications and experience and organizational considerations, such as internal equity. Pay ranges for employees subject to Collective Bargaining Agreements are negotiated by the medical center and their respective union.
