Job Description

Be a part of a world-class academic healthcare system, UChicago Medicine, as a Social Worker II.

 

The REACT Social Worker is part of the violence recovery model at UChicago Medicine that provides psychosocial support via service assessments, case management and supportive advocacy for survivors of community violence post-hospitalization. Specifically, they will provide trauma psychoeducation, ongoing safety risk assessments, service advocacy and person-centered intensive clinical case management in a remote service setting. These interventions will take a holistic trauma responsive social work approach that will include specific interventions such as person-centered motivational interviewing, unconditional positive regard, and harm reduction approaches. This will be done via post hospitalization follow up engagement by supporting the existing REACT program. This is not a therapist position, but rather a blended clinical approach involving primarily case management interventions and advocacy.

 

Essential Job Functions:

 

Trauma Psychoeducation and emotional support:

  • Provide trauma psychoeducation and emotional support for patients to aid in the recovery process after a violent injury and continue rapport building. Refer patients to counseling services when appropriate through private insurance or public agencies.  

Supportive Case Management:

  • Assess case management needs of referred patients and families. Provide ongoing person-centered case management focusing on addressing the social determinants of health and barriers to care. Stay informed of social services available in Chicago and specifically on the South Side that serve individuals and families impacted by violence.

Safety assessments and planning:

  • Ongoing safety planning and assessments done in a harm reduction, person centered, non-judgmental manner utilizing realistic strategies and social supports when needed. Each client engagement should focus on the “goals, psychoeducation and safety” GPS acronym.

Documentation:

  • Document services provided in electronic medical record and program tracking systems.

 

Qualifications:

  • Current LSW in good standing or LSW within 6 months of hire (LCSW preferred)
  • Must have 1-2 years of experience in social service or medical setting (clinical internships are considered appropriate experience)
  • Ability to work flexible hours (primarily daytime hours with occasional weekend)
  • Able to work independently with minimal supervision over daily tasks and assignments
  • Participate in weekly supervision with clinical supervisor
  • Maintain strong working relationships with teammates and medical center staff
  • Ability to adapt to a dynamic service environment
  • Strong interpersonal skills and team oriented collaborative approach
  • Exceptional attention to detail, exceptional organizational skills, punctuality, and record of reliability 
  • Strong understanding of trauma informed care and crisis intervention
  • Extensive experience providing trauma responsive care

Compensation & Benefits Overview

 

UChicago Medicine is committed to transparency in compensation and benefits.  The pay range provided reflects the anticipated wage or salary reasonably expected to be offered for the position.

 

The pay range is based on a full-time equivalent (1.0 FTE) and is reflective of current market data, reviewed on an annual basis. Compensation offered at the time of hire will vary based on candidate qualifications and experience and organizational considerations, such as internal equity. Pay ranges for employees subject to Collective Bargaining Agreements are negotiated by the medical center and their respective union.
