Job Description

Be a part of a world-class academic healthcare system, UChicago Medicine, as a Breast Imaging Nurse Navigator in the Breast Clinic.

 

In this role as a Breast Imaging Nurse Navigator, you will serve as a first contact individual who will assist in guiding new patients with concerning breast imaging on mammography through the appropriate clinical pathways to ensure they receive timely and clinically appropriate workup and evaluation. As a Breast Center Program expert and ambassador, they will provide counseling to patients and families, assist with scheduling imaging, biopsies, and clinical follow up, and demonstrate the mission, vision and values of UCMC. As a Nurse Navigator, this individual will assist in metric tracking, engagement in community outreach, and will work with leadership to ensure compliance with national standards.

 

Essential Job Functions

  • Navigates patients between screening, diagnostic imaging and biopsy to ensure timeliness of care, including placing orders traditionally placed through reflex orders. (Bi-Rad 0, 3, 4, 5, 6)
  • Navigates patients who are new to UCMC
    • Communicates results of Screening Mammogram and High Risk Screener questionnaire to patient
    • Establishes patients who are identified as high risk based on screener into High Risk APP clinic
    • Connects New Patients who schedule a Screening Mammogram through self-referral with a primary care provider if test comes back with abnormal results.
  • Works with interdisciplinary team to coordinate care for patients and contribute to the growth of the Breast Center
    • Coordinates breast imaging follow up care working closely with referring physicians, radiologists, and breast surgeons
    • Identify strategies to decrease barriers to patient access and to make navigator referrals systematic for breast imaging patients.
    • Participates in patient education and post-discharge follow-ups
  • Tracks patient populations to ensure compliance with follow up recommendations and quality assurance procedures in accordance with FDA/MQSA guidelines.
  • Engages in community outreach and education events.
  • Participates and follows up on pertinent tasks related to the Tumor Board discussion

Qualifications

  • BSN
  • Registered Nurse with a current Illinois License
  • Must have general computer skills and be adaptable to changing working situations and work assignments
  • Excellent interpersonal skills required, with specific ability to communicate effectively with patients, family members, and interdisciplinary team
  • Minimum of 1+ year working with Breast Cancer patients
  • Nurse Navigator or Case Management experience preferred

Preferred Qualifications

  • MSN
  • Oncology Certified Nurse or Certified Breast Care Nurse preferred
    • If not a certified nurse, will need to complete Navigation Training

Compensation & Benefits Overview

 

UChicago Medicine is committed to transparency in compensation and benefits.  The pay range provided reflects the anticipated wage or salary reasonably expected to be offered for the position.

 

The pay range is based on a full-time equivalent (1.0 FTE) and is reflective of current market data, reviewed on an annual basis. Compensation offered at the time of hire will vary based on candidate qualifications and experience and organizational considerations, such as internal equity. Pay ranges for employees subject to Collective Bargaining Agreements are negotiated by the medical center and their respective union.
