Job Description

Join UChicago Medicine as aSocial Worker Care Coordinator. Social workers have an integral role in providing the best possible care for every patient during his or her hospital stay and outpatient treatment. Come join our team of committed quality-focused professionals.

 

The Social Work Care Coordination Adult Inpatient (SW CC)), a licensed social worker clinical social work expertise in a given area, plans and monitors consistent patient care delivery. Utilizing a clinical pathway or plan of care the SW CC collaborates with the patient, family, the care team and the multidisciplinary team members to ensure safe, cost effective, timely and efficient care. The SW CC has the responsibility to evaluate the appropriateness of care and facilitate mid-course care changes, in conjunction with the patient care team and multi-disciplinary team members.

 

Additionally, the SW CC will be able to support, teach and act as a resource for all members of the health care team. The SW CC will coordinate and oversee the patient's care primarily during the inpatient phase of treatment. The SW CC will collaborate with other members of the care team and multidisciplinary team as dictated by patient need and appropriate utilization of resources.

 

Essential Job Duties

  • Plans, monitors and coordinates patient
  • Collaborate with the patient, family, care team and multi-disciplinary team to assure appropriate,
  • safe, cost-effective, timely and efficient care.
  • Identifies patient and family needs for consultation with other multidisciplinary specialists and
  • assures needs are met.
  • Plans and conducts or participates in patient care conferences.
  • Assesses patient discharge needs. Assures completion of all discharge-related functions on a timely basis.
  • Develops and implements a plan of care by actively participating in multidisciplinary rounds Assures patient and family educational needs are met.
  • Provides consultation to staff in evaluating current patient status and plan for care.
  • Demonstrates comprehensive knowledge base in area of clinical specialty and functions as a professional role model. Instructs patient and/or families on disease process and appropriate therapy.
  • Provides or assures patient needs are met in adapting to and coping with changing health status. Assures that comprehensive information necessary for the documentation of the patient's hospital course is available to all care team members.
  • Documents in the patient record as appropriate.
  • Provides consultation in specialty area to patient care teams and multidisciplinary team members.
  • Promotes, has knowledge of and utilizes appropriate patient care protocol relating to the physical, psychosocial, and developmental needs of neonatal, pediatric, adolescent, adult and geriatric patients.
  • Serves as a patient advocate for health care issues.

 

Required Qualifications

  • Master's degree in Social Work from a School of Social Work accredited by the Council on Social Work Education
  • Licensed Social Worker (LSW)
  • Self-motivated, organized and comfortable in a fast paced, dynamic environment
  • A Collaborative, solution-focused, team player to help facilitate safe patient outcomes and contribute to institutional goals
  • Previous clinical social work experience in a health care setting of at least one year
  • Licensure in Illinois

 

Preferred Qualifications

  • Licensed Clinical Social Worker (LCSW)
  • Previous clinical social work experience in an acute care hospital setting

Compensation & Benefits Overview

 

UChicago Medicine is committed to transparency in compensation and benefits.  The pay range provided reflects the anticipated wage or salary reasonably expected to be offered for the position.

 

The pay range is based on a full-time equivalent (1.0 FTE) and is reflective of current market data, reviewed on an annual basis. Compensation offered at the time of hire will vary based on candidate qualifications and experience and organizational considerations, such as internal equity. Pay ranges for employees subject to Collective Bargaining Agreements are negotiated by the medical center and their respective union.
