The Social Work Care Coordination Adult Inpatient (SW CC)), a licensed social worker clinical social work expertise in a given area, plans and monitors consistent patient care delivery. Utilizing a clinical pathway or plan of care the SW CC collaborates with the patient, family, the care team and the multidisciplinary team members to ensure safe, cost effective, timely and efficient care. The SW CC has the responsibility to evaluate the appropriateness of care and facilitate mid-course care changes, in conjunction with the patient care team and multi-disciplinary team members.
Additionally, the SW CC will be able to support, teach and act as a resource for all members of the health care team. The SW CC will coordinate and oversee the patient's care primarily during the inpatient phase of treatment. The SW CC will collaborate with other members of the care team and multidisciplinary team as dictated by patient need and appropriate utilization of resources.

Compensation & Benefits Overview
UChicago Medicine is committed to transparency in compensation and benefits. The pay range provided reflects the anticipated wage or salary reasonably expected to be offered for the position.
The pay range is based on a full-time equivalent (1.0 FTE) and is reflective of current market data, reviewed on an annual basis. Compensation offered at the time of hire will vary based on candidate qualifications and experience and organizational considerations, such as internal equity. Pay ranges for employees subject to Collective Bargaining Agreements are negotiated by the medical center and their respective union.
