Job Description

The Clinic Nurse I (RN) CCP Program Nurse will assist physicians in the CCP clinic in patient care activities. They will be responsible for managing clinic flow. They will facilitate care coordination between multiple internal and external resources and also assist in assuring seamless transitions of care between inpatient and outpatient settings. They will be responsible for answering patient phone calls and assisting with medication refill requests and DME (durable medical equipment), and home health service referral requests. They will also manage incoming and outgoing medical faxes and inbasket messages, and stocking patient education materials. CCP nurses need to possess a strong customer service orientation and a commitment to excellence and be highly self-motivated and team oriented while working in a fast-paced, multi-tasking environment with seriously ill and complex patients. They must be able to collaborate with all members of the clinic staff to ensure that patients are treated in a safe, comfortable, and caring environment. This role is completely in person.

 

The Comprehensive Care Program is a primary care clinic that is part of a study looking at models of care for medically complex patients. Today, many patients have a primary care doctor who cares for them in the outpatient clinic. However, if they have to stay in the hospital, they usually get care from a doctor they do not know. This discontinuity in care can worsen patients’ health outcomes and experiences with care, particularly for those with multiple health problems. The Comprehensive Care Program includes two innovative care models that tackle this problem: the Comprehensive Care Physician (CCP) Program and the Comprehensive Care, Community, and Culture Program (C4P). Each offers patients with complex medical and social needs a comprehensive care physician who cares for them in both the hospital and the clinic. We firmly believe that strong relationships between patients and their doctors are key to better health and health care.

 

Essential Job Functions:

  • Performs any or all duties involving direct patient care as assigned by the Clinical Nursing Manager nurse and directed by Nurse Practice Act and Hospital and Patient Services Sector policies and procedures
  • Utilizes the educational assessment of the patient and family which is performed by the R.N. to appropriately implement, evaluate and document patient and family education
  • Observes and records vital signs, assesses patient reactions and reports changes to Licensed Medical Physician
  • Assists the physician with patient examinations or diagnostic procedures
  • Assists with prostate biopsies, cystoscopies and other procedure as needed
  • Assists with the collection of specimens and phlebotomy
  • Documents all aspects of nursing care performed and reflect the critical pathway or the plan of care according to hospital policies and procedures
  • Collaborates with the Licensed Medical Physician in formulating plans of care for assigned patients
  • Interacts with multidisciplinary team members to positively influence patient centered care and focus on problem solving. Works with the patient care team to carry out the agreed upon plan of care

 

Required Qualifications:

  • RN License in state of Illinois
  • Associate Degree in Nursing; BSN degree or higher preferred.
    • Associate Degree Nurses are required to enroll in a BSN program within six (6) to nine (9) months of hire, and make progress toward and complete their BSN degree within 30 months. ADN nurses receive 100% tuition reimbursement towards a BSN degree from an accredited program.
  • 1-3 years of experience as an RN
  • Ambulatory nursing experience preferred
  • Computer proficiency. Familiarity with UCH Information Systems (EPIC, OACIS) preferred.

Compensation & Benefits Overview

 

UChicago Medicine is committed to transparency in compensation and benefits.  The pay range provided reflects the anticipated wage or salary reasonably expected to be offered for the position.

 

The pay range is based on a full-time equivalent (1.0 FTE) and is reflective of current market data, reviewed on an annual basis. Compensation offered at the time of hire will vary based on candidate qualifications and experience and organizational considerations, such as internal equity. Pay ranges for employees subject to Collective Bargaining Agreements are negotiated by the medical center and their respective union.
