Job Description

Join one of the nation’s most comprehensive academic medical centers, University of Chicago Medicine Advent Health as a Post-Acute Care Coordinator.

 

The Post-Acute Care Coordinator leads the on-site management of value-based care contracts (i.e. MSSP, Medicare Advantage) and identified organizational post-acute care (PAC) strategies in the management of patient transitions of care throughout the care continuum. The position oversees a mix of clinical, operational, and business activities related to PAC partnerships, working closely with post-acute providers as well as those we partner with to facilitate high-quality care for our patients in the post-acute space. The position collaborates closely with the inpatient hospital case management department. The position also communicates and collaborates with Physicians and other APPs, Ambulatory Care Coordination, and Community Health staff to ensure a seamless transition of care for our patients.

This position reports to the Executive Director System Post-Acute Care.


Core Duties:

• Manages and works closely with PAC partners in the management of identified patient populations at the four UChicago Medicine Advent Health hospitals. Oversees a mix of clinical, operational, and business activities related to those partnerships.
• Identifies and implements specific program goals including high priority case management redesign efforts required to improve performance in the following areas: Post-Acute Facility utilization; SNF Length of stay (LOS); readmissions reduction as well as reduction in unnecessary ED utilization.
• Works closely with and in partnership with PAC providers, Transition of Care team members, Care Coordinators, and other clinical staff who are integral in ensuring that patients care transitions from acute care to post-acute care are seamless.
• Coordinates and oversees weekly in-person or virtual, multi-disciplinary team meetings with PAC providers
• Travel to various organizational settings and partner facilities, several times per week, is required.
• Coordinates and assists with formal and informal PAQN training sessions
• Assesses, reports, and communicates program status on a periodic basis to all program stakeholders
• Develops and uses a system to continuously evaluate and address internal and external customer concerns/ grievances
• Maintains a log of any quality issues encountered with PAC providers that require follow-up
• Conducts all job responsibilities in coalescence with the organizational mission, vision and values

 

Required Qualifications:
• Must have a bachelor’s degree in nursing from an accredited academic body
• Must be a licensed Registered Nurse with a valid, unrestricted State license

• the ability to lead and manage complex projects that are strategic in nature and scope
• experience/knowledge of transitioning patients from acute to post-acute settings; current clinical knowledge and experience, particularly with the senior population
• the ability to collaborate and motivate other healthcare providers.
• strong/effective problem-solving skills, the ability to be nimble and responsive
• experience with performance measurement, outcome analysis, monitoring tools
• computer skills, specifically Microsoft Office as well as EMRs
• excellent oral and written skills
• excellent organizational skills with demonstrated ability to complete projects in a timely manner
• pro-active and effective team leadership skills.

• Shift: 8-hour shift – 8:00am to 4:30pm – No weekend coverage

Compensation & Benefits Overview

 

UChicago Medicine is committed to transparency in compensation and benefits.  The pay range provided reflects the anticipated wage or salary reasonably expected to be offered for the position.

 

The pay range is based on a full-time equivalent (1.0 FTE) and is reflective of current market data, reviewed on an annual basis. Compensation offered at the time of hire will vary based on candidate qualifications and experience and organizational considerations, such as internal equity. Pay ranges for employees subject to Collective Bargaining Agreements are negotiated by the medical center and their respective union.
