Job Description

Join one of the nation’s most comprehensive academic medical centers, University of Chicago Medicine Advent Health as a Practice Support Coach/Provider Liaison.

As an integral member of the Center for Transformative Care, the Practice Support Coach will work with primary care practices to improve performance in quality of care. Using population registries and related data, Practice Support Coaches work with their assigned practice teams to ensure that patients receive the condition-specific services they need. For example, Practice Support Coaches will: coordinate proactive patient outreach to schedule follow up appointments, screen/monitor patients using questionnaires regarding mental health or other conditions, coach patients as they prepare for cancer screening procedures, and provide reminders about their pre-visit lab tests.

Essential Job Functions:

• Build and maintain strong relationships with network providers, offering support and addressing concerns.
• Educate providers about the clinically integrated network’s goals, services and performance metrics.
• Assist in integrating providers into the network, ensuring alignment with clinical and operational standards.
• Support providers in using health IT systems (such as electronic health records or shared databases) to improve care coordination.
• Serve as a resource for conflict resolution, ensuring that provider concerns are heard and acted upon in a timely manner.


Qualifications:
• Medical Assistant (MA) or equivalent experience
• Minimum of 3 years’ experience in a health care setting. Experience in promoting healthcare behavior change is desirable
• Proficient in data management and reporting
• Proven problem-solver with ability to multi-task
• Prior use of electronic health records and other health care information systems desirable
• Desire to work as part of an interdisciplinary team to improve patients’ health and wellness
• Ability to identify problems, think creatively, and devise innovative solutions.
• Strong interpersonal skills
• Highly organized, proactive and attentive to details
• Ability to work as a member of a collaborative team
• Ability to persuade, influence and enlist others’ support in accomplishing objectives
• Ability to work with large data sets with guidance from medically trained individuals
• Excellent writing and oral presentation skills.
• Experience with computer software, including Outlook, Excel, PowerPoint and Word.

• Job Type/FTE: Full Time (1.00)
• Shift: Day (8:00-4:30PM)
• Unit/Department: Center for Transformative Care
• Work Location: Location Burr Ridge with frequent reimbursed travel to clinics.

In this role, we anticipate that you will generally work approximately 40 hours per week. We anticipate that you will generally be scheduled to work the day shift, with hours between 8:00and 4:30p. The days of the week that you are scheduled will be Monday-Friday, and no weekends.

Compensation & Benefits Overview

 

UChicago Medicine is committed to transparency in compensation and benefits.  The pay range provided reflects the anticipated wage or salary reasonably expected to be offered for the position.

 

The pay range is based on a full-time equivalent (1.0 FTE) and is reflective of current market data, reviewed on an annual basis. Compensation offered at the time of hire will vary based on candidate qualifications and experience and organizational considerations, such as internal equity. Pay ranges for employees subject to Collective Bargaining Agreements are negotiated by the medical center and their respective union.
