Job Description

Join a world-class academic healthcare system,UChicago Medicine Crown Point, located in Crown Point, Indiana as a Transplant Nurse Navigator (RN). UChicago Medicine is an integrated academic and community health system with multiple primary medical facilities within the surrounding suburbs of Chicago.  UChicago Medicine, Crown Point is a new two-story, 130,000-square-foot multispecialty care center and micro-hospital in Northwest Indiana for what will be the academic health system's largest off-site facility and its first freestanding building in Indiana. 

 

As a Transplant Nurse Navigator (RN), you will play a vital role in the care of patients who are being evaluated for a transplant, in the pre-post op and inpatient phases of care and managing care for complex non-transplant liver patients. You will actively collaborate with the multidisciplinary care team. The Nurse Navigator is a specialized nursing role dedicated to managing and preparing transplant candidates for their upcoming surgical procedures and coordinating medical management for advanced liver disease patients. This role ensures that patients are medically, psychologically, and logistically prepared for transplantation. Collaborating closely with multidisciplinary teams, the Nurse Navigator is crucial in facilitating a smooth and efficient transplant process.

 

Essential Job Functions:

  • Travel to outpatient transplant clinics and offsite locations as determined by programmatic needs.
  • Review patient medical histories and perform initial assessments to determine transplant candidacy.
  • Works directly with the multidisciplinary care team and coordinates care for potential transplant recipients.
  • Facilitates offsite testing completion for transplant candidates.
  • Designs patient pre-transplant evaluations including the clinical workup, appropriate diagnostic studies, lab work, nephrology, hepatology, and surgical assessments.
  • Manages outpatient care guided primarily by the transplant team. Triages all outpatient phone calls, guiding patient care through coordination of multidisciplinary aspects.
  • Educates patients and families about the pre-transplantation process, potential risks, post-operative care and follow-up.
  • Develops and/or coordinates the development of patient educational materials and transplant resources.
  • Counsel patients on lifestyle changes, medication regimens, and any preparatory procedures they may need.
  • Manages Epic in-basket messaging for all transplant and liver patients.
  • Collaborates with surgeons, physicians, social workers, and other health professionals involved in the living donation process transplant evaluation process.
  • Coordinates and schedules necessary transplant recipient diagnostic workup, clinic visits, testing and procedures.
  • Completes all United Network for Organ Sharing (UNOS) forms required.
  • Develops and maintains communication and record keeping systems to facilitate patient referral, acceptance, and management within the Transplant Program
  • Participates in regulatory activities including site surveys and certification by UNOS and CMS
  • Participates in patient-related consultations with physician staff as requested, including mandatory attendance at multidisciplinary meetings/rounds
  • Collaborates with team members during patient care meetings by preparing data and maintaining a record of proceedings
  • Acts as the primary point of contact for patients during the transplant evaluation phase of care
  • Advocates for the needs and concerns of patients, ensuring they receive appropriate care and attention

 

Required Qualifications:

  • Associate Degree in Nursing; BSN degree or higher preferred.
    • Associate Degree Nurses are required to enroll in a BSN program within six (6) to nine (9) months of hire, and make progress toward and complete their BSN degree within 30 months. ADN nurses receive 100% tuition reimbursement towards a BSN degree from an accredited program.
  • Current licensure as a Registered Nurse in the State of Indiana.
  • Minimum of 2-3 years of nursing experience, with a focus on Transplant, Critical Care, or a related specialty.
  • Active BLS certification from the American Heart Association (AHA)

Compensation & Benefits Overview

 

UChicago Medicine is committed to transparency in compensation and benefits.  The pay range provided reflects the anticipated wage or salary reasonably expected to be offered for the position.

 

The pay range is based on a full-time equivalent (1.0 FTE) and is reflective of current market data, reviewed on an annual basis. Compensation offered at the time of hire will vary based on candidate qualifications and experience and organizational considerations, such as internal equity. Pay ranges for employees subject to Collective Bargaining Agreements are negotiated by the medical center and their respective union.
