Job Description

Join University of Chicago Medicine, as a Clinical Staff Pharmacist in the Outpatient Pharmacy Mail Order Call Center.

 

In this role as a Staff Pharmacist, you will assume responsibility and accountability for the provision of safe, effective, and prompt medication therapy. Through various assignments within the department, they provide support of centralized and decentralized medication-use systems as well as deliver optimal medication therapy to patients with a broad range of disease states. Clinical Pharmacists adeptly function in pharmacy operational areas, including but not limited to central dispensing pharmacies, satellite dispensing pharmacies, and specialty pharmacy areas. 

 


Responsible and accountable for the provision of safe and effective drug therapy and other pharmaceutical care services intended to achieve outcomes related to the cure, diagnosis or prevention of disease, elimination or reduction of patient’s symptoms, or arresting or slowing of a disease process, and improve a patient’s quality of life in collaboration with a multi-disciplinary team. The pharmacist also assumes the responsibility for pharmacy technicians that work directly under his/her supervision by ensuring the proper preparation of drug products and managing the workflow.

 

Essential Job Functions

 

  • Supervises pharmacy technicians working under their direction to provide the following and assure completion of:
    • Triaging the order of the department workflow as required
    • New or refill prescription order entry
  • Ensure the accuracy and appropriateness of all medication orders by evaluating drug selection, dose, frequency, safety, dosage form, allergies, drug interactions, and disease state interactions prior to dispensing of medications.
  • Ensure the accuracy and appropriateness of all medication orders prior to dispensing by evaluating patient information including (but not limited to) age, weight, relevant medical characteristics, and laboratory results.
  • Demonstrate the knowledge and skill necessary to provide care appropriate to the age of assigned patients.
  • Demonstrate knowledge of the principles of growth and development over the patient’s life span and assess data reflective of the patient’s status.
  • Identify service requirements by interaction and consultation with patients, physicians, nurses, pharmacists, and technicians.
  • Ensuring that all weight-based medication regimens are appropriate for individual patients based on disease, organ function, height and weight, body surface area calculations, age, etc. Exercises professional judgment in evaluating the safety and efficacy of all medication orders written for this patient population.
  • Document clinical interventions which prevent or eliminate drug-related problems (i.e. Adverse drug reactions, drug interactions, etc.) or promote cost-effective drug therapy.
  • Provide consultation for assigned patient populations and as requested by the medical staff. Educate patients on safe and effective use of medications, possible side effects of their medications, the management of these side effects and proper disposal of medications.
  • Clarify and solve problems concerning medication orders with prescribing physicians, suggest alternative medication therapies to physicians when appropriate, sere as a source of information for investigational drug information for health professionals.
  • Maintain and enhance up-to-date professional knowledge of drug and drug therapy by attending educational meetings and reviewing relevant professional journals and suggested readings.
  • Performs other duties as requested by senior management.
  • Assist in the orientation and training of new personnel.

 

Required Qualifications 

 

  • Graduate of an ACPE accredited School of Pharmacy Doctor of Pharmacy or another advanced degree preferred.
  • Registered pharmacists in the state of Illinois or eligible for same.
  • Two to five years of retail/outpatient pharmacy experience preferred. Exceptional aptitude or knowledge may be considered in lieu of experience.
  • Knowledge of contemporary retail/outpatient pharmacy/clinical practice and service. Strong written and verbal communication skills.
  • Ability to interact constructively with supervisors, peers, support personnel, and students. Knowledge and understanding of retail/outpatient pharmacy operations.
  • Ability to integrate clinical services, distributive services, education, and research activities
