UChicago Medicine Medical Group seeks an exceptional Hospice and Palliative Care Advanced Practice Nurse (APN) for UChicago Medicine Medical Group.

 

Job Description Summary:

The APN works in collaboration with Palliative Care physicians and other members of the Palliative Care Team in a consulting role and in active, clinical practice to direct and facilitate the care and support of patients and families who are facing life-altering illness.  Services and care provided are defined by patient care needs.

 

Hospice and Palliative Care Advanced Practice Nurse (APN) Roles and Responsibilities:

  • Performs comprehensive initial assessment of patient’s medical, psychological, social, emotional, and spiritual needs.
  • Implements evidence-based pharmacologic and/or non-pharmacologic interventions to alleviate pain distress based on diagnosis and constraints of the illness.
  • Reassesses patients to evaluate effectiveness of symptom management and revise goals of care and treatment plan as needed.
  • Involves patient, family, health care team members, and appropriate community resources in the plan of care throughout the illness trajectory.
  • Establishes effective trust relationships with patients and families and incorporates supportive and therapeutic counseling skills when indicated.
  • Promotes ethical decision-making while respecting patient autonomy with regard to expected outcomes, end-of-life care situations and advance care planning.
  • Serves as a clinical resource and nursing expert on palliative care and promote excellence in clinical staff through coaching, mentoring and role modeling.
  • Demonstrates leadership abilities in areas requiring communication, collaboration, cooperation and coordination to achieve optimal patient outcomes.
  • Acquires and maintains current knowledge and competency related to best practices / evidence based practices in palliative care.
  • Assists in developing, reviewing, evaluating and revising standards of care and/or agency protocols.
  • Establishes a method to document and evaluate the quality and effectiveness of palliative care nursing practices.
  • Commits to one hundred (100%) patient and customer satisfaction by always exhibiting a courteous and helpful manner during interactions with others, including patients, families, visitors, physicians, students, and co-workers.

Position Qualifications:

  • Current state of Illinois nursing license as an RN and an APN.
  • Current Advanced Practice Nurse certification.
  • 3 – 5 years’ experience in hospice and palliative care; at least one-year experience as an APN.
  • Current certification in Hospice and Palliative Care Advance Practice Nurse by NBCHPN or certification must be obtained within one year of hire.
  • Demonstrated outstanding diagnostic, interpersonal, leadership and organizational skills.
  • Working knowledge of the ANA Scope and Standards of Hospice and Palliative Nursing Practice.
  • Eligibility to apply for National Provider Identifier Number, Illinois Mid-level Provider Controlled Substance license, and Federal Drug Enforcement Administration Mid-level Practitioner registration.
  • Membership in at least one national professional nursing organization, HPNA preferred.
  • Current CPR certification required.
  • Possesses and maintains a valid driver’s license and current automobile insurance and has the availability of personal and dependable mode(s) of transportation to conduct home visits.
  • This position requires the ability to perform medium work, exerting up to 50 pounds.
