Job Description

Be a part of a world-class academic health-care system at UChicago Medicine within our NCI-designated Cancer Center as a Cancer Program Nurse Navigator. In this role, you will collaborate with the entirety of the multi-disciplinary Breast Cancer program to provide patients the highest quality of care. As a navigator focused on breast cancer, you will serve as the outpatient clinical liaison and a single point of contact for patients. You will be responsible for coordinating the care of patients as they move from initial diagnosis through survivorship. You help ensure timely delivery of care and eliminate potential barriers, identify resources, and serve to enhance breast cancer disease management and the patient experience.

 

In this role, the Cancer Program Nurse Navigator is responsible for providing education and emotional support to patients and families, in addition to connecting patients with needed community and hospital resources. As an integral member of the breast cancer team, the Cancer Program Nurse Navigator will communicate any pertinent issues related to the patients’ clinical, emotional, spiritual, psychosocial, financial, and other needs to the appropriate clinicians and staff.

Essential Job Functions:

  • Assesses clinical, emotional, spiritual, psychosocial, financial, and other patient needs
  • Acts as point of contact and liaison for patients, families and their multidisciplinary clinical team, and ensures treatment plan progression
  • Facilitates resolution to patient needs and monitors follow up
  • Educates/reinforces education with patients and families on disease, treatments, side effects, adverse reactions, and reportable signs/symptoms based on multidisciplinary care plan
  • Works with patients and family members to ensure discharge instructions are understood and followed
  • Coordinates development of patient education materials, clinical pathways, and quality studies in partnership with other team members
  • Coordinates and/or participates in multidisciplinary consultations and/or tumor conference
  • Documents data and navigation assessments as needed for accreditation and program development
  • Documents recommendations made at multidisciplinary consultations and/or tumor conferences and follows up when appropriate for coordination of care
  • May be responsible for direct patient care within the scope of the role
  • Performs other duties as assigned

Required Qualifications

  • Registered Nurse with a current Illinois license
  • Graduate of a National League for Nursing Accredited School of Nursing with BSN
  • Minimum of two (2) years clinical experience working as an RN in a medical, radiation, or surgical oncology capacity or as a patient/nurse navigator within oncology
  • Demonstrated clinical competence in patient care, teaching and management of patients with a cancer diagnosis, and knowledge about clinical aspects of cancer and its prevention, diagnosis, and treatment
  • Excellent interpersonal skills required, with specific ability to communicate effectively with patients, family members, physicians, advanced practice providers, and other members of the clinical and administrative teams
  • Demonstrated ability to self-start and self-motivate

Preferred Qualifications

  • Oncology Certified Nurse preferred
  • Nurse Navigator Certification (AONN, ACS; other) preferred
  • Familiarity with Radio-Therapeutics treatments preferred
  • Familiarity of local, state, and national cancer awareness, education, outreach, and advocacy programs and organizations is preferred
