In this role you will partner with the rest of the clinical team to provide patients with the highest quality of care. Our Care Coordination team delivers the highest quality of care while ensuring continuation of care for our patients is a seamless process.
The Case Manager is a member of the leadership team charged with developing and maintaining patient care standards. They will coordinate and oversee the patient's care primarily during the inpatient phase of treatment. The Case Manager will collaborate with other members of the care team and multidisciplinary team as dictated by patient need and appropriate utilization of resources.
