Job Description

UChicago Medicine Medical Group seeks an exceptional Palliative Care Nurse Practitioner for our inpatient hospice integrated practice unit at Ingalls Memorial Hospital in Harvey, IL. 

 

The Advanced Practice Nurse will provide comprehensive palliative care to patients and work collaboratively within a multidisciplinary team, using advanced clinical skills to manage a caseload of patients in a community based physician practice. Assess, diagnose, initiate delegated medical treatments, perform independent nursing interventions and evaluate care. Provide a model of care that is shaped by theory, expanded by science, research and reflective practice and is identified by a holistic and comprehensive approach to the patients.

 

Essential Job Functions:

  • Independently manage the care of acutely ill patients within area of expertise (Palliative
  • Care) and within parameters of medical staff privileges.
  • Obtain relevant patient health history.
  • Study and interpret patient tests and records.
  • Perform physical exam and record findings.
  • Prescribe/order appropriate pharmacologic and non-pharmacologic interventions and
  • develop a treatment plan
  • Develop an educational plan for patient/family.
  • Determine effectiveness of treatment plan with documentation of treatment provided and
  • patient outcomes.
  • Reassess and modify plan with patient/family and treatment team within the parameters of
  • medical staff privileges.
  • Assistance with diagnosis, treatment, and management of acute and chronic health
  • conditions.

  • Assistance with care coordination for palliative care referrals.
  • Conduct initial palliative care consult visits and follow up palliative care visits in the inpatient and outpatient settings as assigned

 

Required Qualifications:

  • Advanced Practice Nurse/Nurse Practitioner
  • Graduated from an accredited Nurse Practitioner program and hold current certification
  • 3+ years of previous APN palliative care experience preferred, but not required
  • Active Illinois licensure
  • Must be eligible for DEA registration.
  • Previous APN experience preferred, but new graduates will be considered
