    Shift: Full-Time (1.0) / Day Shift / 8-hr Shift

    In this role, the New Patient Associate serves as a point of contact for the coordination of outside medical records for patients in select programs prior to the patient’s first appointment. The New Patient Associate works with patients/family/caregivers to obtain outside medical records, and diligently tracks acquisition of records. The New Patient Associate ensures the timely and comprehensive collection of records and appropriately transfers records to the clinical program or physician prior to the patient’s appointment. The New Patient Associate provides escort services for new patients and other administrative support as needed.


    Responsibilities include:

    • Use program/physician specific guidelines to determine medical records retrieval needs. As needed, contact patients, outside medical offices and hospitals to acquire appropriate records.
    • Assist patients requesting records in accordance with institutional policies, HIPAA, and the statutes for the State of Illinois.
    • Accurately complete requisitions for submission of records.
    • Track all information gathered, submit, and provide follow up on needed information in a timely and professional manner.
    • Deliver records per protocol to appropriate provider prior to appointment.
    • Prepare new patient packets and send prior to the patient’s appointment.
    • Maintain the patient’s calendar to ensure all new patients are escorted to the appropriate ambulatory practice.
    • Follow the Standards of Behavior and promote the Ideal Patient Experience. Utilize customer service standards to promote patient satisfaction in all operations. Protect each patient’s privacy.
    • Maintain current knowledge of the ambulatory practices and the hospital campus.
    • Greet patients in a warm and professional manner.
    • Coordinate with New Patient Coordinators, Practice Administrators/Managers, and the Patient Services Center as needed to ensure continuity of patient care to meet outlined expectations.
    • Perform other duties as assigned.

    Required Qualifications:

    • A completed high school diploma or equivalent from an accredited educational body is required.
    • Must have excellent interpersonal skills and demonstrate their ability to work cooperatively in a team environment.
    • Must demonstrate strong customer service skills.
    • Must have 3 or more years of experience working in a customer service role in a healthcare environment.
    • Must be competent working with Microsoft Office Word.

    Preferred Qualifications:

    • Experience working in an ambulatory practice is preferred.
    • Working knowledge and experience using EPIC or other electronic medical records systems is preferred.
