    We're hiring a Care Coordinator for the CMO Office in a Full-Time, Day Shift at the University of Chicago Medicine in Hyde Park, Chicago, IL. Please note this role includes traveling to our additional facilities located throughout the South and Southwest city and suburbs.

As a Care Coordinator, you will work on a multidisciplinary team including physicians and payers to ensure the patient’s progress in the acute episode of care through post discharge, is quality driven by cost effective and resourceful goals. The Care Coordinator works alongside attendings and consulting physicians to facilitate effective patient transition through the process of hospitalization.


    The Care Coordinator works collaboratively with a multi-disciplinary team to ensure patient needs are met and care delivery is coordinated across the continuum, as well as appropriately reimbursed by payers as contracted. In this role, you will seek the expertise of social workers in an effort to resolve psycho-social patient care issues, and develop complex patient transition and/or discharge plans as needed. This individual will interact with patients, family members, healthcare professionals, the Chicagoland community and state agencies. The Care Coordinator serves as a liaison between the hospital and community agencies or facilities for the exchange of clinical and referral information. The Care Coordinator is responsible for maintaining hospital compliance with the Quality Improvement Organization (QIO) series and CMS guidelines. In addition, the Care Coordinator provides case review information to third party payers, assists in the denial and appeals process, and assesses quality, levels of care and identifying and reporting potential risk management issues. The incumbent performs duties and tasks in accordance with performance standards established for the job. They are also responsible for participation in, and completion of, all patient safety initiatives appropriate to the position. The Care Coordinator conducts all job responsibilities in coalescence with the University of Chicago Medicine PRIDE values.


    Care Coordinator Education and Experience Requirements: 

    • Must have a Bachelor’s degree in Nursing from an accredited academic body.
    • Must be a licensed Registered Nurse with a valid, unrestricted State license.
    • Must have a minimum of five years' experience working in a clinical setting as a Registered Nurse.
    • Must have experience working in case management or care coordination.
    • Certification in Case Management preferred. (i.e. CCM, CCCTM, etc.)
