    Case Manager - RN, the Risk, Patient Safety, & Compliance department, Registry - PRN Days, University of Chicago Medicine in Hyde Park, Chicago, IL 

     

    We’re hiring a Case Manager – RN to work Registry hours for our Risk, Patient Safety, & Compliance department with the University of Chicago Medicine in Hyde Park, Chicago, IL.

    The Registry Case Manager (CM), a registered nurse with clinical expertise in a given area, plans and monitors consistent patient care delivery. Utilizing a clinical pathway or plan of care the CM collaborates with the patient, family, the care team and the multidisciplinary team members to assure safe, cost effective, timely and efficient care. The CM has the responsibility to evaluate the appropriateness of care and facilitate mid-course care changes, in conjunction with the patient care team and multi-disciplinary team members. Additionally, the CM will be able to support, teach and act as a resource for all members of the health care team. This position will be responsible for working at least 2 weekend days/month, preferably available to cover one weekend day most weekends. Also, will be utilized for covering planned time off and surge units. The Registry Case Manager is a member of the leadership team charged with developing and maintaining patient care standards. The Case Manager will coordinate and oversee the patient's care primarily during the inpatient phase of treatment.  The Registry Case Manager will collaborate with other members of the care team and multidisciplinary team as dictated by patient need and appropriate utilization of resources.

     

     

    Case Manager - RN Education and Experience Requirements: 

    • Must have a Bachelor’s degree in Nursing from an NLN accredited school of nursing; a Master’s degree in Nursing or a related field is preferred.
    • Must be an actively licensed Registered Nurse in the state of Illinois, or eligible to pursue licensure within the state of Illinois.
    • Being certified in Case Management is preferred.
    • Must have a minimum of 3 years of Nursing experience, 1 years in leadership is preferred.
