    In this role, the New Patient Nursing Coordinator uses clinical judgement while serving as the first point-of-contact for patients and physicians to coordinate services with UCM medical professionals, ancillary services, BSD, and hospital offices/staff.  Triages patient after gathering disease history, prior treatments and current state of health.  Facilitates efficient and effective processes for patients to access various services within the hospital.  Provides guidance to patients through the evaluation process, works with various departments to ensure patients complete their itineraries, revises and communicates itinerary changes as required. 


    Responsibilities include:

    • Completes intake history and uses clinical judgement and triage tools to determine patient appointment needs. Serves as first point-of-contact for patients and physicians to coordinate services with UCMC medical professionals, ancillary services, BSD, and hospital offices/staff.  Provides quality assurance on records related to diagnosis and clinical history to coordinate and manage educational/advisory meetings, evaluation tests, and consultations for patients within specified time periods. Acts as a clinical resource to New Patient Coordinators.  Educates patient on visit expectations and provides basic counseling on subspecialty services.  Immediately and independently resolves issues encountered during the day to keep the patient on schedule. Provides advance notice to patients and referring physicians of required follow-up testing to maintain regulatory compliance and coordinates the required follow-up. Collaborates with various departmental staff to ensure proper insurance and billing information is provided for patient’s services.
    • Ensures clinical protocols and triage tools meet standards for each specialty program. Provides periodic quality assurance review on records obtained for appointments.  Utilizes databases to maintain patient data, maintains statistical data, and provides detailed reports (charts, graphics, tables, etc) to management on patient services. Creates and organizes mailings as applicable to regulatory agencies.  Manages the day-to-day operations for the collection and transliteration of primary source documentation for regulatory reports for patients. Responds to inquiries from regulatory agencies, resolves issues/discrepancies and prepares correspondence to ensure the proper and timely transfer of appropriate information.  Periodically conducts “after visit calls” to ensure patients had questions answered and know how to contact their clinical team and clinic as their care continues.
    • Generates and distributes program materials, welcome packets, educational materials, appointment itineraries and various notification letters per protocols.
    • Triages patients, responds to referring physicians, UCMC medical professionals and case manager requests. Independently determines the appropriate course of action.  
    • Handles confidential information consistent with Federal HIPAA legislation and all other applicable federal, state, local, and institutional regulations with regard to confidentiality
    • As applicable; facilitates referral, pre-certification and authorization process as required for initial encounter. Screens patients for supportive care services, including dietitians, socials workers and financial counselors. Directs patients and families to available community resources and supportive services.  Assists patients to overcome barriers to care, e.g., transportation, childcare, financial counseling.


    Required Qualifications:

    • Three years’ experience in cancer care preferred.
    • Experience in patient advocacy preferred.
    • Experience working with various Microsoft based computer software programs for Word required, experience using Mac preferred.
    • Knowledge of scheduling templates, insurance verification, and third-party collection procedures.
    • Knowledge of hospital billing and collection processes. classes offered by the Medical Center after hours or on weekends, with advance notice.


    Preferred Qualifications:

    • Bachelor of Science in Nursing
