    Ambulatory Clinical Documentation Specialist, CMO Office, Full-Time, Day Shift, University of Chicago Medicine in Hyde Park, Chicago, IL

     

    We're hiring an Ambulatory Clinical Documentation Specialist for the CMO Office in a full-time, day Shift at the University of Chicago Medicine in Hyde Park, Chicago, Illinois!

     

    The Ambulatory Clinical Documentation Specialist (CDS) is responsible for facilitating improvement in the overall quality and completeness of ambulatory medical record documentation on a concurrent basis. The CDS promotes modifications to medical record documentation to ensure accurate depiction of the level of clinical services provided to the patient and to completely describe patients’ severity of illness. The CDS obtains appropriate clinical documentation through collaborative communications from having developed positive working relationships with physicians, nursing staff, other patient care providers, and the Health Information Management Department Coders.

     

    The CDS is also responsible for providing informal and formal education to all members of the care team regarding clinical documentation. The outcome will be documentation that accurately and completely captures the clinical picture/severity of illness/complexity of the patient while providing specific and complete information to be utilized in coding, profiling and outcomes reporting of both the facility and the physicians. The CDS utilizes knowledge of national coding guidelines (ICD-10 and CPT), Hierarchical Condition Categories (HCC), standards of compliance, and clinical knowledge to identify opportunities and to achieve results.

     

    Required Qualifications:

    • License/ Certification: Registered Nurse
    • BS degree minimum.  Masters preferred
    • 5 -8 years progressive work experience;Case Management or Med/Surg or Critical Care Nursing experience preferred.
    • Basic computer skills in word processing and spreadsheet utilization
    • Excellent interpersonal skills necessary to develop and foster effective physician communication
    • Analytic skills necessary to clinically assess medical records
    • Excellent prioritization and organizational skills
    • CCS coding credential strongly preferred.
    • Knowledge of documentation requirements and coding guidelines that pertain to professional procedure and diagnosis coding.
