We're hiring a Community Health Worker Supervisor for the CMO Office in a full-time, day Shift at the University of Chicago Medicine in Hyde Park, Chicago, Illinois!
The University of Chicago Medicine’s (UCM) Center for Transformative Care has implemented a Community Health Worker (CHW) program. The CHW program will work with UCM patients across neighborhoods in Chicago’s South Side and South Suburbs to address social determinants of health (conditions in the environments in which people are born, live, learn, work, play, etc. that affect a wide range of health outcomes and risks). CHW’s will assist UCM patients with health specific education, navigation and access to community services and other resources, and self-managing health conditions and adopting improved health behaviors.
The Community Health Worker Supervisor will be responsible for supervising and supporting the day-to-day functions of a team of CHWs; responsibilities include, but are not limited to: health education, patient follow-up, resource advocacy/linkages, and outreach activities to individuals, families and community member. Some of the education will take place in the homes of community members; however the CHW may also be required to provide education in other settings such as group sessions, health fairs, clinics, etc. as needed. The CHW Supervisor will evaluate the CHW’s competencies in each of these settings and provide feedback. The CHW Supervisor will also be responsible for assisting with the organization of community health promotion activities and events, as well as program publicity and recruitment. The CHW Supervisor will serve as the primary or secondary CHW across the various CHW programs and will assist in the program design and planning process to execute newly developing programs and refine existing programs.
